Zollinger-Ellison syndrome is characterised by severe, recurrent peptic ulceration, gastro-oesophageal reflux and diarrhoea. It results from the overproduction of gastric acid, due to production of the hormone gastrin by a secretory tumour, or 'gastrinoma'. These rare neuroendocrine tumours are most commonly found in the duodenum or pancreas. Most gastrinomas arise sporadically but in a quarter of cases occur in association with multiple endocrine neoplasia type 1 (MEN1 syndrome). In this article, Andrew Robert Moore, Andrea Varro and Mark Pritchard outline the diagnosis and treatment of gastrinomas and the associated Zollinger-Ellison syndrome. The symptoms of this condition can now usually be well controlled with acid-suppressing drugs; hence, prognosis is primarily determined by tumour-related factors.
